Infinity Smile Dental Hygiene Studio is independently owned and operated by Catherine Taccogna, a Registered Restorative Dental Hygienist (RRDH) regulated by the College of Dental Hygienists of Ontario. We respect your privacy and are committed to protecting your personal and health information.

As a regulated health professional providing health care in Ontario, we handle your personal health information in accordance with Ontario’s Personal Health Information Protection Act, 2004 (PHIPA). Where PHIPA does not apply, such as to information collected through this website for business purposes, we follow Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA).

This policy explains what information we collect, how we use and share it, how we protect it and what choices you have.

We collect only the information we need to provide safe, appropriate dental hygiene care and to run the studio. This may include:

• Contact and identity details: your name, phone number, email address, mailing address and date of birth.
• Health information: your medical and dental history, medications, allergies, health conditions, notes from your appointments, assessments, treatment records and any images or X-rays taken.
• Insurance and billing details: information needed to submit claims to your insurer and to process payment.
• Communications: messages you send us by phone, text, email or through online forms, including appointment requests.
• Website information: basic technical data such as your browser type and pages visited, collected through cookies and similar tools (see Cookies below).

We collect this information directly from you, or from another health care provider or your insurer with your consent or as permitted by law.

We use your information to:

• provide, document and follow up on your dental hygiene care;
• book, confirm and remind you of appointments;
• communicate with you about your care, including referrals;
• bill you or your insurer for services;
• meet our professional, legal and regulatory obligations, including record keeping;
• improve the quality of our services and our website.

We will not use your personal health information for purposes unrelated to your care, such as marketing, without your consent. You may withdraw consent to receive marketing messages at any time.

We do not sell your personal information. We share it only when necessary and permitted by law, for example:

• With other health care providers involved in your care, such as your dentist or physician, when a referral or coordination of care is needed.
• With your insurer when you ask us to submit claims or direct bill.
• With service providers that help us run the studio, such as our online booking, website hosting and email providers, who are expected to protect your information.
• With the College of Dental Hygienists of Ontario or other authorities when required for regulatory purposes or by law.
• When required or permitted by law, such as to reduce a serious risk of harm.

Within the health care team, your information may be shared on the basis of implied consent for the purpose of providing your care, unless you tell us otherwise. You may ask us to limit how your information is shared, and we will explain any limits on doing so.

We take reasonable steps to protect your information from theft, loss and unauthorized access, use, disclosure, copying or modification. These include physical, technical and administrative safeguards appropriate to the sensitivity of the information.

Records are kept for the length of time required by the College of Dental Hygienists of Ontario and applicable law, and then securely destroyed.

If your personal health information is lost, stolen or accessed without authority, we will notify you and the Information and Privacy Commissioner of Ontario as required by PHIPA.

Under PHIPA you have the right to:

• be informed about how your information is collected, used and shared;
• give, refuse or withdraw your consent, subject to legal limits;
• ask to see and obtain a copy of your health record;
• ask us to correct information that is inaccurate or incomplete;
• ask questions about our privacy practices; and
• make a complaint if you believe your privacy rights have not been respected.

To make a request, contact us using the details below. We will respond within the time required by law. If you are not satisfied with our response, you may contact the Information and Privacy Commissioner of Ontario. For matters under PIPEDA, you may contact the Office of the Privacy Commissioner of Canada.

Our website is hosted on Squarespace and may use cookies and similar technologies to make the site work, remember your preferences and understand how visitors use it. Some pages show content from other providers, such as an embedded Google map or Google reviews, which may set their own cookies.

You can control or delete cookies through your browser settings. Blocking some cookies may affect how parts of the website work. We do not use cookies to collect your health information.
